Payment & Accepted Insurances

Accepted Insurance Providers:

* Blue Cross

* Regence Blue Shield

* First Choice Health

* Aetna

* Medicare (limited spaces available)

* Medicaid (Obstetrical care and children under the age 18)

Please have an up-to-date insurance card on hand at the time of your first appointment.  Please also notify the clinic of any changes in your insurance coverage as soon as possible to prevent receipt of non-covered bills at your residence.

Co-pays:

Our contracts with the insurance companies require us to collect co-pays at the time of service.  Additionally, there are other services (such a circumcision & vasectomy) that we will require payment in advance.

Patients Without Insurance Coverage:

Payment at the time of service is required.  [Sliding scale fee schedules are available in some circumstance.]  Short-term payment plans are available, but must be requested prior to the services being performed.
